Operator stated the lid has been open during lunch. Operator removed raw beef and placed in walk-in-cooler to chill. Rechecked beef at end of inspection at 43F-46F Also, ice cream mix (46F - Cold Holding) in ice cream machine at bar area. Operator stated it was working fine earlier. Operator was checking the setting on machine. Rechecked ice cream mix at end of inspection at 44F-45F
Food-contact surface soiled with food debris, mold-like substance or slime. Underside of shake machine in bar area soiled. Also, blade on can opener in prep area soiled.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in warewashing area and none at hand wash sink in prep area. Operator replenished paper towels at both hand wash sinks.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm). Operator manually primed machine and ran a couple of cycles. Retested at 100ppm.

Raw fruits/vegetables not washed prior to preparation. Observed unwashed avocados on cook line with stickers still on them. Manager had employee remove stickers and rewash avocados.
