Handwash sink not accessible for employee use at all times. Handwash sink at the bar is inaccessible due to location, beer tap cooler is in the way.
Water treatment device has not been inspected or serviced according to manufacturer's instructions. Observed no date marked on water filter to the ice machine stating when it was last changed.
Bowl or other container with no handle used to dispense food. Observed no handle bowl in use in bulk sugar bin to scoop sugar.

Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee crack eggs then use a spatula with washing hands and changing gloves. Reviewed process of cracking raw shell eggs and proper handwashing.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touch toasted bread with bare hands, explained the proper method options for handling ready to eat food. Operator discarded toast.
Handwash sink not accessible for employee use at all times. Observed handwashing sink at bar area blocked by refrigerator and table.
Employee with no hair restraint while engaging in food preparation. Observed employee preparing food on the cook line with no hair protection.

Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed build up of dust like substance on ceiling tile vents.
Employee eating in a food preparation or other restricted area. Observed bottle of water with a twist cap on the the right hand side of the cook line next to steam well food warmer.
Equipment in poor repair. Observed torn gaskets on reach in double door freezer on service line, and also on glass door reach in cooler at the bar area.
In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored on drip grate of soda fountain in between uses.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop not stored properly inverted or hung to dry.
