Operator is not properly tracking/marking the number of days opened commercially processed, ready-to-eat, time/temperature control for safety food was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed opened tomato sauce in plastic container date marked 8/5/025, per operator items frozen the day after and thawed today at 8/20/2025, observed no date marking of sauce frozen and thawed at refrigeration temperature again. Advised operator to provide proper date marking and tracking for food thawing.
Floor soiled/has accumulation of debris. Floor soiled in back kitchen area near ice machine.
