Dead roaches on premises. Observed 1 dead roach behind prep table in kitchen. Operator discarded, cleaned and sanitized the area.
Employee with no hair restraint while engaging in food preparation. Observed employee that was handling prepped food, with no hair restraint.
Floor soiled/has accumulation of debris. Observed floors along all walls of the establishment are soiled.
No handwashing sign provided at a hand sink used by food employees. Observed 2 handwashing sinks with no signs. Provided them to the operator and they were hung during inspection.
Outer openings of establishment cannot be properly sealed when not in operation. Observed both rear doors with gaps at the top of the doors.
Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Observed exposed wires and studs on wall under the bar hand sink.
