Menu contains an item made with a raw/undercooked animal food as an ingredient and description of item does not inform consumer of raw/undercooked animal food ingredient. Cold smoked salmon on buffet not indicated to be raw, per operator served raw. Operator printed a sign to indicate raw food.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed self service buffet properly time marked on chart with 4 hour discard, no written plan. Provided operator with time as a public health control form and explained usage to operator.
Single-service articles improperly stored. Boxes single service cups and napkins on floor in back dry storage hallway.
