Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee changed his gloves without washing his hands before putting on a new pair of gloves. I informed the manager and he informed the employee.
Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee used his cellphone with gloves on and the prepared food. I informed the manager and he had the employee to remove the gloves.
Raw animal food stored over/not properly separated from ready-to-eat food. Raw fish over Cole slaw in the top section of the flip top reach in cooler across from the fryers in the kitchen. The Cole slaw was removed. Fish over cut lemons in the top section of the flip top reach in cooler across from the fryers in the kitchen. The lemons was relocated.
Establishment not maintaining clam/mussel/oyster tags for 90 days.

Cardboard used to line food-contact shelves. Cardboard lining the shelves for dry storage.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. The fish was removed from the packaging.
Food stored on floor. Bags of onion inside the walk in cooler. Case of oysters inside the walk in freezer.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Manager washed gloves instead of removing the gloves. Informed the manager gloves are not to be washed; but removed wash hands and put on a new pair of gloves.
Toxic substance/chemical improperly stored. Spray bottles with degreaser, bleach water and glass cleanser stored above the prep table in the dishwasher area. The products was removed.
Handwash sink used for purposes other than handwashing. Lid to a pan was rinsed at the hand washing sink in the kitchen. The lid was taken to the 3 compartment sink to be washed, rinsed and sanitized.
Spray bottle containing toxic substance not labeled. 2 spray bottles on ledge above the prep table. One with a lime green substance and one with water that the manager stated was bleach water. The bottles was labeled.
Current Hotel and Restaurant license not displayed.
Employee with no hair restraint while engaging in food preparation. Operator was cooking food without a hair restraint.
Food stored on floor. Bulk container of breading inside the walk in cooler. 2 cases of shrimp on the floor inside the walk in freezer. All products was removed from off the floor.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Handle on the scoop inside the Cole slaw the handle was removed from out of the Cole slaw.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Stainless ladle and a white slotted spoon in a container of water at 79F on the prep table next to the rice cooker in the dishwasher area. The water discarded and the utensils was washed, rinsed and sanitized.
Light not functioning. Hood system
Time/temperature control for safety food thawed in an improper manner. Red snapper,cod tilapia and shrimp thawing in standing water inside the 3 compartment sink. The manager put the product under cold running water.
