Observed 2 employees working in the establishment for approximately 5 months without being certified as food handlers.
Cutting board has cut marks and is no longer cleanable. Observed white cutting board with excessive marks on top of cooler at the coffee station.

Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed server using his phone with gloves on, then with the same gloves touched the bagel and plated it. Employee washed his hands and manager discarded the bagel.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed carrot cake kept on the top of the front counter at 60 F, as per manager for less than 4 hrs, cakes are stored inside walk in freezer and take out as needed. Carrot cake was sent to walk in cooler immediately.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed missing backflow preventer at the mop sink hose Bibb connection, splitter was added. Hose was connected at the time of the inspection.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Sanitizer concentration 0 ppm.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed cook discarded a broken shell eggs and then put gloves to continue working with food without washing his hands. Employee was coached and washed his hands correctly.
Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed dishes inside the dishwasher machine, but chlorine sanitizer was 0 ppm. Employee set up 3 compartment sink with 200 ppm.
