Accumulation of black/green mold-like substance in the interior of the ice bin located in the back of the kitchen. The operator cleaned the interior of the ice bin.

Observed Shredded Mozzarella Cheese (48F - Cold Holding) in pizza make line reach-in cooler stacked above fill line. Operator moved to different part of cooler. Product had been in cooler for less than 4 hours.

Observed chlorine sanitizing dishmachine at 10 ppm. Operator contacted service company to repair machine.
Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Opened bag of raw shrimp stored over opened bag of ready-to-eat french fries in freezer. Operator stored correctly and voluntarily discarded french fries.

Roach activity present as evidenced by live roaches found. Observed 5 lives roaches in crack of bent cove molding along wall underneath three compartment sink to the right of the dishmachine. Operator sprayed roaches and they are now dead. Operator contacted pest control company and they are in route to the establishment. Operator cleaned and sanitized area. Pest control arrived and is further treating and reviewing the area.
Dead roaches on premises. Observed 1 dead roach behind ice machine on top of water filter. Operator removed, cleaned, and sanitized area.
