Hole in or other damage to wall. Observed wall around electrical outlet on back wall next to exit door with hole around it.

Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. Observed no knowledge of the big 6. Handed operator a copy to post.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chemical test kit for their quaternary triple sink.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels in the establishment besides the restroom hand wash sink.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no signed agreements. Had operator sign a copy and gave them more.
Spray bottle containing toxic substance not labeled. Observed a spray bottle containing chemicals unmarked in the kitchen. Operator labeled the bottle.
Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Operator had triple sink set up as wash,sanitizer,rinse. Operator emptied the sanitizer and filled it correctly.
Food stored on floor. Observed cases of drink mix and water bottles on the floor of the establishment. Operator put items on a shelf.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed an employee drink in the reach in cooler. Operator removed the drink.
Single-service articles not stored inverted or protected from contamination. Observed coffee filters and to go containers stored unprotected and un-inverted. Operator inverted the containers.
