Dishwasher handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Spoke with employee, employee properly washed hands before returning to work.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed server garnish beer bottles with lime wedges using bare hands. Spoke with operator regarding glove use when garnishing.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In tall silver reach-in cooler raw ground beef (45F - Cold Holding); raw scallops (45F - Cold Holding); raw shrimp (45F - Cold Holding); raw chicken (45F - Cold Holding); raw beef (45F - Cold Holding) for 3 and a half hours per operator. Operator placed all foods in ice bath. In 15 minutes all foods dropped to 42F or below.
Employee personal food not properly identified and segregated from food to be served to the public. In clear sliding door reach-in cooler in middle of kitchen, observed employee foods mixed in and above food for public.
