Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On prep table next to dough mixer, butter (63F - Cold Holding). Employee stated it has been out for approximately 1 hour. Inspector went over time as a public health control with employee and employee labeled butter pan for four hours. Inspector sent time paperwork to establishment.
Toxic substance/chemical improperly stored. Spray bottle of degreaser stored on dry storage shelf next to slicer equipment. Employee removed degreaser from shelf.

In reach in cooler on cook line, pan of ricotta cheese and Alfredo sauce not date marked. Employee stated both are pulled from freezer and put in pan directly onto cook line. Ricotta cheese was put in cooler Sunday and Alfredo sauce was put in cooler Saturday. Employee put date marks on products.
Food-contact surface soiled with food debris, mold-like substance or slime. On clean dish rack, cutting boards (red, white and green) heavily stained with black. Also, wooden pizza board peeling.

Toxic substance/chemical improperly stored. On top shelf of prep table next to cook line, 12 volt battery stored next seasonings. Employee stated the battery was just changed and the old one put on shelf. Employee discarded.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where green hose is attached. Also, vacuum breaker missing on threaded faucet in garbage enclosure, no hose attached.
Food-contact surface soiled with food debris, mold-like substance or slime. On clean dish rack across from triple sink, multiple cutting (white, green and red) have black stains.
Spray bottle containing toxic substance not labeled. On rack to left of triple sink, two spray bottles of purple liquid not labeled. Employee stated it is all purpose cleaner and will labeled once other employee clocks in.

Only employee working answered phone (taking order) at front counter and went into kitchen, placed gloves on to prepare food without washing hands. Inspector coached employee on proper hand washing procedures.
