Both front counter employees were on cell phones then put on gloves and began working with food - one made frozen smoothie, the other made a customers coffee without first washing hands. Reviewed proper hand washing and employees properly washed hands. Third front counter employee entered prep area and began making customer coffee without first washing hands. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Front counter low boy- smoked salmon (52F). Per operator item placed in unit approximately 3 hours ago. Item not prepped or portioned today. Operator moved item to freezer in Mezzanine kitchen to quick chill. Front counter - glass merchandiser - quiche (50-52F); overnight oats (50-52F); cooked eggs (49F). Per operator items placed in unit approximately 3.5 hours ago. Operator placed on Time as a Public Health Control. Provided operator form and reviewed.
