Per operator, out of temperature for approximately 15 minutes. Operator returned to thermalizer to reach 165+F.

Observed Employee on personal phone, put phone away, then handled container from microwave with food for customer with no glove change. Operator educated employee on proper procedures for hand washing and glove change.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Walk in cooler: sliced chicken (63F - Cooling) at 9:50 since 8:05 - 62F 10:10 Observed in covered plastic containers. At current rate of cooling product will not reach 41F within 4 hours. Operator placed in freezer to quick chill.
Wet wiping cloth not stored in sanitizing solution between uses. Observed Wiping rag resting on prep counter by reach in cooler at food assembly line. Operator returned to sanitizer bucket.
