Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced cheese (52F - Cold Holding); cooked beef brisket (48F - Cold Holding); raw ground beef (46F - Cold Holding) in reach in cooler #4. The operator provided a temperature log showing temperatures less than 40F taken at 12:00. The operator moved the above foods to the walk in cooler.
No conspicuously located ambient air temperature thermometer in holding unit #4. The operator provided a thermometer for the unit.

Employee with no hair restraint while engaging in food preparation. Observed employee walking on to line to start his shift without a hair restraints on. Observed employee putting on hair restraint and being coached by operator.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed cooler on front counter with soiled gasket.
Silverware/utensils stored upright with the food-contact surface up. Observed silverware stored on drying rack handles down. Observed operator removing silverware, and putting into dish dishwasher to rewash, then store properly.
