Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shelled eggs stored over ready to eat sweet and sour sauces in reach-in cooler in prep area. Operator removed raw shelled eggs to properly store.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed half and half (46F - Cold Holding) in reach-in cooler on front service line. Operator stated the carton was placed in there today. Operator removed and placed in nearby cooler until cooler able to maintain temperature of 41F and below. Also, cheese danish (44F - Cold Holding) in front display cooler on front counter. Door way not fully closed. Operator lowered temp of cooler to chill danish
Handwash sink used for purposes other than handwashing. Observed ice dumped in hand wash sink at front service line. Operator melted ice and cleaned hand wash sink.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Menu board and to go menus have Vietnamese Egg Coffee without a disclosure that egg is raw egg yolk. Operator stated she will have menu board and to go menus changed by her spouse.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach-in cooler on front service line unable to maintain temperature of 41F and below. Ambient temp of cooler at 57F. Observed large mass of ice over fan.
Dead roaches on premises. Observed one dead roach under shelf in prep area. Operator cleaned it up and discarded.

Clean equipment/utensils not stored at least 6 inches above the floor. Observed slicer in prep area not stored at least 6 inches above floor.
Single-service articles improperly stored. Observed box with wrapped sleeves of single service plastic bowls and lids stored on floor next to bread oven and aluminum pans on shelf near dishwasher not inverted. Operator removed to properly store single service items and inverted aluminum pans on shelf above dishwashing machine
Standing water in floor drain/floor drain draining very slowly. Observed standing water in drain under triple sink. Drain started to drain slowly. Operator plunged drain during this inspection and all water drained.
Time/temperature control for safety food thawed in an improper manner. Observed meat in triple sink thawing in pan of standing water. Operator turned water on to continue thawing properly.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Operator working at front service line making coffee, then went to kitchen area and placed gloves on without washing hands. Also, employee touching phone in office, then put gloves on the make sandwich. Discussed with operator.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Raw beef stored over ready to eat ham in reach in freezer in kitchen area- not all products commercially packaged. Operator removed raw beef to properly store.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. liver pate (58F - Cold Holding) stored in top of reach-in cooler above chill line of cooler. Operator placed in bottom of cooler to chill. Rechecked at end of inspection at 45F
Toxic substance/chemical improperly stored. Container of Lysol wipes stored next to single use cups on shelf below front counter. Also, container of Clorox wipes stored next to single use cups and aluminum foil on top of ice machine. Operator removed Lysol wipes and Clorox wipes to properly store.
Handwash sink not accessible for employee use at all times. Hand wash sink at front counter blocked by dish tack full of cups and miscellaneous utensils. Also, hand wash sink next to mop sink blocked by pans and containers being stored in hand wash sink. Operator cleared hand wash sink at front service line.
No proof of required state approved employee training provided for any employees. Operator unable to provide proof of employee food safety training during this inspection.
