Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Chef entered kitchen and started to handle a pan with ground beef, without washing his hands first. Chef went to wash his hands.
Food-contact surfaces in contact with time/temperature control for safety food not cleaned at least every four hours. Dirty container holding ground beef patties in drawer under grill, since yesterday Chef placed ground beef in a clean container.
