Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple Sink (Chlorine 0ppm); Triple Sink (Quaternary 200ppm). It was a chlorine bucket that was not reading. The operator changed it with a new quaternary bottle.

Whole fruit displayed for self-service not wrapped and no utensils provided. Observed apples stored in dining area in a grab and go setting not protected and no dispensing tongs present for item. Discussed proper protection methods with operator.
1 dead roach on premises to the left of the ice machine. Operator removed, cleaned, and sanitized area.
Plumbing system in disrepair at handwash sink at front entrance of kitchen. Cold water handle won't turn cold water off.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed high heat sanitizing dishmachine at less than 160F at dish level. Employee placed items in three compartment sink to sanitize.

A temperature was not taken today and the items had been in the cooler overnight.
