Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Inside refrigerated make-table at cookline: shrimp (48F - Cold Holding). Employee stated the shrimp was thawed under running water approximately 30 minutes prior and added ice to rapidly cool them to 41F.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. 1. Backflow prevention device missing at hose connection near mop sink. 2. Backflow prevention device missing at hose connection near outdoor bar area. 3. Backflow prevention device missing at two spigots near dumpster.

Handwash sink used for purposes other than handwashing. Red sanitizer bucket stored down inside handwash sink in server station. Employee removed bucket from sink during this inspection.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Air vent on ceiling in kitchen near cookline soiled with black substance.

Time/temperature control for safety food thawed in an improper manner. Container of frozen shrimp thawing on table at room temperature, near kitchen entrance. Employee moved shrimp to sink to continue thawing under cool running water.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On ice on counter in server station: butter packets (61F - Cold Holding); sour cream (58F - Cold Holding). Employee stated she placed the butter and sour cream packets in the ice one hour prior. Employee added more ice to top of items to cool to 41F.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Fish thawing inside reduced oxygen packaging inside refrigerated make-table at cookline. Inspector had employee open packages during this inspection.
Old labels stuck to food containers after cleaning. Painters tape stuck to side of pan on clean dish rack near dish machine.

Inspector printed a consumer advisory and server posted it during this inspection.

Thawed hotdogs missing date mark inside reach-in cooler drawer below grill. Employee stated the hotdogs were thawed on 04/05 and added proper date mark during this inspection.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Container of pasta missing date mark inside reach-in cooler at cookline. Employee stated the pasta was cooked on 04/05 and added proper date mark during this inspection.

Employee stated the butter was placed into ice bin approximately 15 minutes prior. Inspector had employee add water to ice to surround container of butter to help lower temperature to 41F.
