Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black build up on the ice machine.
Single-service articles not stored inverted or protected from contamination. Observed coffee filters and to-go containers stored improperly in the dry storage area. Operator disposed of the coffee filters and inverted the to-go containers.
A return visit. The problems from the previous inspection had been dealt with.
Ordered shut on the spot. 2 violations recorded, 1 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Rodent activity present as evidenced by rodent droppings found. Observed approximately 50 rodent droppings on the floor of the back left storage area. Observed approximately 12 rodent droppings on the floor of the dry storage area. Observed approximately 3 rodent droppings on the floor of the food prep area. Observed approximately 20 rodent droppings on the floor of the front counter area. Operator has started sweeping and mopping the areas.
Cardboard used to line food-contact shelves. Observed cardboard lining a snack shelf in the front counter. Operator removed cardboard.
A return visit. The problems from the previous inspection had been dealt with.

Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in the reach in freezers in the back kitchen area and the front counter area.
Unnecessary items/unused equipment on the premises. Observed unused and broken down equipment in the back kitchen area: two auto fryers,one standing reach in cooler, one sandwich reach in cooler, and one ice cream machine.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw burgers stored beer ready to eat bread in the reach in cooler in the front counter area. Operator disposed of burgers.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Cheese pizza and Pepperoni pizza hot holding at 110 degrees in the back prep kitchen. Employee had stated they had been out for approximately 1hr. Employee reheated pizzas to 165 degrees.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed the quaternary sanitizer in the three compartment sink at 500ppm. Employee added more water to the mixture and remeasured it at 300ppm
Food service manager not certified within 30 days of employment. Food-contact surface soiled with food debris, mold-like substance or slime. Observed food debris on front counter soda machines. Observed food debris build up on meat slicer in back kitchen area.
No probe thermometer provided to measure temperature of food products. Employees stated they did not have a probe thermometer.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no signed employee agreements. Printed out a agreement and had the operator and an employee sign the agreement. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed prepared peanut butter sandwich on counter not date marked.

Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed triple sink used in the incorrect order as they were washing, sanitizing, then rinsing. Employee re-setup the triple sink to the correct order.
