By the front counter at sushi station, observed cooked rice (98F - Hot Holding) without a time mark. Operator stated rice has been out at ambient temperature for less than 30 minutes, used for an order. Operator time marked rice.

Warewashing sanitizing solution exceeding the maximum concentration allowed. At time of inspection Triple Sink (Chlorine 200+ppm). Operator diluted with more water. End result Triple Sink (Chlorine 100 ppm).
In-use utensil stored in standing water less than 135 degrees Fahrenheit. At time of inspection, Knife used for sushi was placed in lukewarm temperature water. Operator dumped water, wash and sanitized knife for use.

Bowl with no handle used to dispense flour. Operator removed the bowl.

Spray hose at dish sink lower than flood rim of sink. In kitchen, observed spray hose nozzle inside utility sink. Operator hung nozzle above the sink
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed a three way splitter connected to three hoses with no vacuum breakers installed. Educated operator about anti-siphon devices and safety.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance inside ice machine. Operator cleaned and sanitized area.
Propane tank (larger than 2.7 lb. water capacity/1 lb. gas capacity) located inside of the building. For reporting purposes only. Observed 20 pound lp gas tank hooked up to the wok in the kitchen
