Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee put on gloves and begin to scoop ready to eat ice cream and handle clean equipment without washing hands first.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed jug of milk sitting on front prep counter at 72F-cold holding, per employee items not prepared or portioned today and left out of temperature for approximately 1 hour. Milk placed back in reach in cooler to quick chill

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed only quaternary ammonium chemical test kit inside establishment soiled/ completely saturated in blue liquid substance no longer functional to properly test sanitizer.
