Observed server bring back dirty dishes, add to dish tray, then handle clean plate and syrup container without hand wash. Also, observed cook crack shell eggs, then handle clean plate without handwash. Spoke to employees during inspection.

Observed cups of sour cream stored higher than ice in pan. Operator stated sour cream was put in ice about two hours prior. Operator removed sour cream and placed in nearby reach-in cooler
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing from non chemical side of splitter at mop sink.
