Live, small flying insects found Approximately 10 small live flying insects at bar landing on blender motor, discussed with operator to clean and sanitize unit and eliminate all flying insects.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In walk in cooler raw chicken stored above raw ground beef, discussed with operator who corrected storage of products.

Live, small flying insects found. Approximately 3 live, small flying insects found in kitchen on cook line landing walls/storage shelves. Approximately 2 live, small flying insects found in kitchen dish washing area. Advised Operator to eliminate live, small flying insects, clean/sanitize areas.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At upright stainless reach in freezer; raw fish stored over opened package/plastic wrapped container of cooked vegetables. Operator moved raw fish to bottom shelf.
