Observed employees numerous times changing tasks leaving cook line taking orders from customers at front counter and returning to cook line to cook foods; employee grabbed customers cup of coffee to dump the excess inside handwash sink and returned to cook line and started to cook eggs; manager educated and employee changed gloves and washed hands;
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking- cheese bagels at front counter and back of the kitchen on the speed rack no time marked; operator time marked 8:00 am
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Chlorine over 200ppm) operator changed to 100 ppm.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At Dining Room; self service ultra pasteurized half and half (46°F - Cold Holding). Operator stated being held 1 hour, moved to reach in cooler.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. At front line flip top cooler; blended cream cheese (53°F - Ambient Cooling 9:50am/55°F - Ambient Cooling 10:20am). Operator stated cream cheese prepped 2.5 hours prior, at this rate product will not reach 41°F within 4 hours. Pan overfilled, operator removed overfilled portion and put in reach in cooler.
Floor soiled/has accumulation of debris. At walk-in freezer; floor has accumulation of debris.
Food not stored at least 6 inches off of the floor. At walk-in freezer; raw chicken inside cooler stored on floor. Operator removed from floor.
