Handwash sink not accessible for employee use at all times. Observed handwash sink blocked by cutting board. Operator removed cutting board.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator has form and just needs to complete. Operator had employees present sign employee agreement before leaving inspection.

Carbon dioxide/helium tanks not adequately secured.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 5 packages of raw salmon not removed from packaging with instructions to remove before thawing. Stop Sale. Operator discarded salmon.
Food not stored at least 6 inches off of the floor. Observed box of frozen raw chicken stored on floor in walk in freezer underneath shelving.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed quaternary ammonium concentration at 100 ppm.
