In the reach in cooler next to cook line, observed cut tomatoes (54F - Cold Holding); cut lettuce (53F - Cold Holding); shredded cheese (55F - Cold Holding). Operator stated foods are moved from walk in cooler to the reach in cooler around 10:00am.

In the refrigerated drawers on the cook line, raw chicken and raw beef was stored above raw shrimp. Educated Operator and cooks about raw storage and proteins were all moved to proper storage hierarchy for food safety.
Hole in or other damage to wall. Missing wall tiles/hole in wall behind the dishwasher.
Plumbing system in disrepair. Handwashing sink at entrance to the kitchen is loose and no longer attached completely to the wall.
