Ripped/worn tin foil used as shelf cover. Observed tin foil wrapped bottom shelf of prep table in dishwashing area ripped.
Ordered shut on the spot. 10 violations recorded, 3 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine measuring (chlorine 0ppm). Manager on shift confirmed establishment does not have 3 compartment sink to utilize manual washing.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken stored above raw pork in walk in cooler. Manager corrected storage.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed a mold like substance on the ice drop tray inside the ice machine.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed the ceiling vents in the kitchen rusted and soiled.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the hood filters and the piping with a grease build up.

Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed the shelves in the walk in cooler rusted and in the back kitchen.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed ground beef at 50f and cheese at 49f in the top drawer of under grill drawer. Manager stated product had been in drawer around 90 minutes. Manager placed product on ice.
Ceiling tile missing. Observed corner of ceiling tiles bowed but missing above kitchen hand sink.
Ceiling/ceiling tile shows damage or is in disrepair. Observed some ceiling tiles bowed but no water present.

Equipment in poor repair. Observed legs and bottom shelf of prep sink pitted with rust. Observed leg of dish machine pitted with rust.

Ceiling/ceiling tile shows damage or is in disrepair. Observed two small holes in kitchen ceiling where pipes enter the ceiling.
Equipment and utensils not properly air-dried - wet nesting. Observed plastic pans wet stacked on shelf next to dish machine.
Hole in or other damage to wall and ceiling. Observed hole/ opening in wall in back kitchen where soda lines go into wall. Owner plugged hole and taped over until permanent repairs could be completed.
