Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Ran dish machine multiple times, tested 0ppm. Operator prime sanitizer manually to cleared air gap in tube, ran dish machine again and tested 50ppm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Large reach-in cooler on cook line: cooked chicken 45 F, raw beef burger 52 F, raw chicken 47 F. Operator moved to reach-in freezer for rapid cooling. Re temped: cooked chicken breast 39 F, raw beef burger 43 F, raw chicken 41 F.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee walked outside and returned to the kitchen, touched the waste food debris collector under the dish machine, then engaged to prep food at cook line without changing gloves and washing hands. Discussed with employee, employee discarded soiled gloves, washed hands and replaced gloves.
Toxic substance/chemical improperly stored. Sanitizing bucket with quaternary ammonium stored in the cold well with other food items on reach-in cooler at cook line. Employee removed sanitizing bucket.
Accumulation of black/green mold-like substance in the interior of the ice machine.
