Operator is the only certified food manager. Operator states they renewed it but was not able to show proof.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chemical test kit for their chlorine triple sink. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no signed agreements. Had operator sign a copy and gave them more.
