Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Sitting in warmer unit on make line in kitchen, chicken breast (95F - Hot Holding, retest 167F). Operator stated chicken was sitting there less than 1 hour. Operator chicken back into fryer to get temperature up.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior sides of ice machine in storage room has build up of mold like substance.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. All ceiling vents in kitchen are soiled with dust.
Duct tape used to repair nonfood-contact surface. Gaskets of right sided reach in cooler in back storage room of kitchen is torn and repaired with tape. Handle for door of same cooler has tape.
Employee with no hair restraint while engaging in food preparation. One employee engaged in food prep does not have on hair restraint.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Mac and cheese sauce (67F - Cooling, after two hours 49F), in large metal covered pot in walk in cooler. Manager removed lid and placed in reach in freezer.

No paper towels or mechanical hand drying device provided at handwash sink. Hand wash station by cook line, employee corrected during inspection.
