Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Two employees started working in kitchen, wear gloves, without washing hands first. After instructed, employees discarded gloves and washed hands.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee placed both bare hands inside container of flour. Manager instructed, employee washed hands and put in new set of gloves.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. mash potatoes (50F - Cold Holding); cooked white rice (48F - Cold Holding), per manager food out of WIC less than one, hour. Observed food containers next to steam table, food out to reheat for late afternoon customers. Advised operator to placed back on WIC, and to have out when will be immediately reheated.
