Quaternary ammonium sanitizer used to sanitize clean in place equipment not at proper minimum strength. Do not use equipment not properly sanitized. Observed 0 ppm for Quat buckets. Operator and inspector dumped buckets and replaced with new sanitizer; final reading: 200 ppm.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter with no breaker.
