22 live small flying insects found in kitchen, prep area, and bar landing on reach-in cooler, liquor bottles, ice bin, and dry storage shelves. Operator started to spray to eliminate flies.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Rice (48F - Cold Holding) in reach-in cooler on cook line across from grill. Operator moved to another cooler.
Toxic substance/chemical improperly stored. Observed multiple areas where toxic chemicals were stored above dry storage items and single service items. Operator moved and stored correctly.
Condensation or other drainage not disposed of according to law in ceiling in prep area leaking into trash can, underneath ac dripping on to prep table, and above door leaking from ac in rear of kitchen. Operator placed buckets to catch dripping water. Operator has a service order in for repair.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. One air curtain in bar is currently not in operation and live flies are present in the bar. The other aid curtains are fully functioning and on at time of inspection. Operator repaired.
