Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Dishwasher failed to wash hands before handling clean sanitized dishes after immediately rinsing and loading dirty dishes. Manager reviewed with employee proper procedures and employee washed hands.
Raw animal food stored over/not properly separated from ready-to-eat food. In prep kitchen walk in cooler, open case raw shell eggs above open cases of baguettes. Manager rearranged to properly separate.
Handwash sink not accessible for employee use at all times. Park of quaternary wipes in buffet hand sink. Manager removed.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee in prep kitchen wearing wristbands. Employee removed.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed chick peas salad (59F - Cooling) since 10:30am, at 1:10pm. 59F at 1:30pm; inside 3 door cooler. Foods stored covered, inside cooler. Operator removed covering and put foods on ice.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Sink and Surface 0ppm). Operator corrected to 700ppm.
A return visit. The problems from the previous inspection had been dealt with.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed accumulation of black mold like substance on soda gun nozzle at bar. Bartender cleaned and sanitized during inspection.
Ceiling tile missing. Observed ceiling tile missing above clean dish rack in kitchen.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulation of dust on ceiling tile above prep table in kitchen.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed plastic containers not inverted on clean dish rack in kitchen.
Utensils in poor condition. Observed several rubber spatulas chipped and torn, stored above triple sink in kitchen. Operator voluntarily discarded.
Nothing was found. The inspector recorded no violations at all.
