Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black build up on the ice machine. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust build up on multiple ceiling vents in the kitchen.
