Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the microwave ceiling has an accumulation of dried food debris. This was wiped off by staff.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed : the reach-in freezer has ice buildup on all shelves.
Open dumpster lid. Observed : the dumpster lid was open, no one around.
Soiled cloths, linen, aprons, coats, or other uniform apparel not kept in a suitable container prior to laundering. Observed : staff apron, used, stored with dry goods on the dry goods shelf. This was moved below.
A return visit. The problems from the previous inspection had been dealt with.

Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed butter packs in the counter top reach-in cooler on the buffet, 59-60°. These had been left out during service today. The intent was to reuse then again tomorrow.

Toxic substance/chemical improperly stored. Observed: a gallon jug of Easy Off oven cleaner stored on top of the plastic egg dishes in the kitchen cabinet. The cleaner was moved to the chemical shelf.
No certified food manager for establishment. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Observe the surfer food measure card was not available for review.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed: a chlorine test kit was not found for when chlorine is being used.
Equipment and utensils not rinsed between washing and sanitizing. Observed the 3 compartment sink was not set up to wash-rinse-sanitize, they were out of quaternary amonia sanitizer. Inspector suggested chlorine bleach, which was available. The 3 compartment sink was then set up correctly. Quaternary amonia has been ordered.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the area above the juice nozzles are soiled.
