Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker connecting hose attachment to mop sink.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed opened carton of milk in walk in cooler without date mark. Per manager opened on 03-21-24. Manager date marked milk during inspection.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed accumulation of debris inside soda nozzles at dining area.

Clean utensils or equipment stored in a bathroom, locker room or mechanical room. Observed box of food service disposable cups stored in employee bathroom. Manager removed cups from bathroom during inspection.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed tuna in reduced oxygen packages with label indicating to be removed prior to thawing at cook line make table. Manager removed tuna from packaging during inspection.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed two employee beverages stored on prep tables at prep and cook line. Manager discarded beverages during inspection.
