Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed beans on metal pot on stove with no heat on beans (105F - Hot Holding) Cook states on stove for 2 to 3 hours. Heated food to 165F within 2 hours to place on line.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Found at 0ppm sanitizer bucket empty. Dishwasher replaced sanitizer and retested to 50ppm
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee walk-in in from outside cooler area put wet wipe cloth on handsink then add gloves and prepare foods on line without washing hands.
