Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed by the triple sink. The manager installed a vacuum breaker.
Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Observed the sanitizer bucket with chlorine at 500 ppm. The manager poured out the liquid rinsed out the towels and filled with quaternary at 300 ppms.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed the ice chute soiled. The manager removed and washed, rinsed,and sanitized the chute.
Equipment and utensils not properly air-dried - wet nesting.
