Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed yellow rice at 123F left above of the flat top. As per cook, for less than 1 hr. Employee decided to move the rice to reach in cooler and cool down to keep it cold.
Establishment charging automatic gratuity/ operations charge without properly disclosing the information on the menu and the bill not visible or smaller than the letter size used for food items.
Bowl or other container with no handle used to dispense food. Observed container with no handle touching scoop the flour. Employee stopped using it.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed mold like substance on reach in cooler gaskets ( cooler used for ketchup)
Wet wiping cloth not stored in sanitizing solution between uses.

Stop Sale issued due to food not being in a wholesome, sound condition. Observed ROP fresh fish conducted on site using regular bags instead of 10K bags. Fish was discarded.
Establishment packaging time/temperature control for safety food using a reduced oxygen method that does not control the growth of and toxin formation of C. botulinum and the growth of Listeria monocytogenes. Establishment does not have an approved Process Waiver from the Division of Hotels and Restaurants. Observed establishment conducting ROP without approved process waiver ( storing food inside vacuum sealed bags ).

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed chicken above salmon inside walk in cooler main kitchen. Employee inverted them.
Exterior door has a gap at the threshold that opens to the outside. Observed a gap underneath the exterior door of the ware washing area.
Plumbing system improperly installed or repaired. Observed one of the hand sink at the outdoor bar is not properly installed. As per Director of Facilities, a new one is coming to replace it.
Wiping cloth sanitizing solution stored on the floor. Observed sanitizing solution in the bucket stored on the floor at the bar. Employee moved it immediately.

Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed establishment is offering raw items without identified them on the menu. Person in charge started printing new menus while I was conducting the inspection.
Establishment increased seating without providing written approval from the local fire authority and wastewater provider. Observed license was approved for 35 seats, establishment submitted projected amount of 251 seats which is subject to local authority having jurisdiction approval. TCO only, but not CU approved at the time of the inspection.
Nothing was found. The inspector recorded no violations at all.
