Utensils in poor condition. Observed: water filters next to the ice maker, not maintained per labeled instructions, change by 3-23-25.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. ice cream mix (49F - Cold Holding) item came from the walk in cooler and was placed in reach in cooler, employee stated reach in cooler was turned off so she turned it on. Employee placed ice inside unit to help rapidly chill food item.
No paper towels or mechanical hand drying device provided at handwash sink. At Handwash sink next to tray makeup area. Manager placed towels in dispenser.
Clean equipment/dishware/utensils stored below soap or paper towel dispenser exposing the items to drips from people's hands. Milkshake collars under towel dispenser and hand sanitizer dispenser by office area.
