Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Observed lights towards back of kitchen not shielded.

Employee with no beard guard/restraint while engaging in food preparation.
Floor soiled/has accumulation of debris. Observed black buildup under shelving and along cove base in walkin cooler. Very Strong mildew smell inside cooler. Floor soiled in walk-in freezer with old food debris.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed old food debris on interior of makeline reach in cooler.
A return visit. The problems from the previous inspection had been dealt with.

Toppings containers are stored within the broken drop well. Edges around well has been broken away and cherries and other food toppings are ground into broken area. Observed black growth on plastic components and dispensing spout of slushie machine.
Floors not maintained smooth and durable. Observed deep grout lines on flooring throughout the establishment and water and food debris standing in grout lines on warewash and storage areas and at front of store corner of entryway..

Observed employee handle keys and touch eyewear then use hand sanitzer and proceed to touch single service items and scoop ice without properly washing hands.

Handwash sink used for purposes other than handwashing. Observed two dish scrub pads stored in handsink at front 3 bay sink at wait prep area. Manager had area cleaned. And handsink emptied.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Form was on paperwork but not filled out. Observed operator had a copy of the Health agreement but had not implemented the action.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed two employees with expired training.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee scooping open ice and handling single service items with filigree and stone rings. Other employees with multiple rings,multiple items on wrists and watches. Manager addressed employees had them remove jewelry and wash hands.
Floor area(s) covered with standing water. Observed dirty water standing on floor at wall below breakers at end of cookline near warewash area. Water standing along wall. Old food debris within the water.
Floor soiled/has accumulation of debris. Observed old food on floors at warewash area. Also Old food and trash debris outside front door at front entrance.
Floors not maintained smooth and durable. Observed deep grout lines on flooring throughout the establishment and water and food debris standing in grout lines on warewash and storage areas and at front of store corner of entryway..
Food stored on floor. Observed boxes of whipped topping and cases of soft serve mix stored on floor in walk-in cooler boxes o FF stored on floor in walk-in freezer. Truck came last the day before.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice across floor and on condenser in walk in freezer. Floor buckling from ice. No longer smooth and easily clean able.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed lemon presses stored in standing water at 93F at drink prep frontline. Manager removed and put through dishwash.
Old food stuck to clean dishware/utensils. Observed pans at warewash dry rack with food still adhering to the surface.
