Under-grill drawers: hamburger (51F - Cold Holding); cheese (52F - Cold Holding); tomatoes (49F - Cold Holding); pork (51F - Cold Holding); hot dogs (46F - Cold Holding). Manager stated drawers had been opened often during the prior lunch rush hour. All food moved to walk-in cooler for faster cooling. Ambient on drawers per equipment thermometer was 37F. Wait station: slaw (46F - Cold Holding).
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee speaker on shelf over back prep table.
Food stored on floor. Cases of bread on floor in walk-in freezer. Manager moved to shelf.
In-use tongs stored on equipment door handle between uses. Tongs on cookline equipment handle protruding into walkway. Manager adjusted to side.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Soda machine backsplash soiled with mold-like substances.
Old labels stuck to food containers after cleaning. Label residue on clean pan stored on shelf in warewashing area.
Working containers of food removed from original container not identified by common name. No label on container of rice on shelf in back prep area.

Metal stem-type thermometer soiled. Observed : the thermometer used on the cook line was soiled with dried food debris. This was then cleaned.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink. The vacuum breaker at the top of the splitter can be placed on the 'non chemical' side of the splitter.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the nozzle in the drive thru has build up. ALSO, the wait station side machine has nozzles with build up.
Compartment of equipment subject to the accumulation of moisture is not sloped to drain to allow complete drainage. Observed : the beer cooler at the front wait station has an accumulation of water.
Hole in or other damage to wall. Observed on partial wall in the mop sink closet is not sealed.
In-use tongs stored on equipment door handle between uses. Observed : tongs on the cook line, protruding into the walkway. These were turned sideways to comply.
Water leaking from pipe and/or faucet/handle. Observed : the drain pipe for the ice maker is draining onto the floor.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed : egg rolls on the cook line, 70°. They were sitting out from before the 4 pm shift came in (undetermined time)The manager chose to discard.
Carbon dioxide/helium tanks not adequately secured. Observed : the CO2 tanks at the pony keg holders were not adequately secured. The chain was then wound around the collars to secure them to each other.
Employee personal food not properly identified and segregated from food to be served to the public. Observed : there is a reach-in cooler at the pickup station that is not identified for employees only. The manager on duty said he would make a sticker to identify the whole reach-in cooler as for employees only.
Wood food-contact surface not properly sealed. Observed : the plywood at the pickup station is not sealed.

Soiled dry wiping cloth in use. Observed a dry cloth that had an accumulation of food debris on it.
Standing water in bottom of reach-in-cooler. Observed: the pony keg beer cooler with standing water inside.
Water leaking from pipe and/or faucet/handle. Observed the mop sink faucet was on and the hose was leaking at the connection, down the hose and on to the floor. Mop sink was turned off and the water stopped leaking.

Toxic substance/chemical improperly stored. Observed: blue window cleaner stored on the bottom shelf in the drive thru, next to to go containers. The spray was relocated to cleaning.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed: no back flow were required in these areas: by outside by the smokers, outside by the back door and on the non chemical side of the splitter for the mop sink.
Bowl or other container with no handle used to dispense food. Observed the scoop for the sugar on the cook line does not have a handle. The scoop was removed.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed the chef base drawers all temperatures above 52. Drawers may have been left open was the only answer given.
Floor area(s) covered with standing water. Observed: standing water in the dish pit. ALSO, at the back door.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the exterior of the wait station soda machine near the restroom, foreign substance on the right side lip.
Single-service articles stored on a soiled surface. Observed: to go clam shells stored on top of the cook line hot hold cabinet.
Worn, torn and/or soiled floors/carpeting. Observed: next to the drive thru soda machine, an accumulation of debris between the wall and soda machine.

Buildup of food debris/soil residue on equipment door handles. Observed the cook line handles with an accumulation of food debris. A paper towel was employed to start removing the food.
In-use knife/knives stored in cracks between pieces of equipment. Observed on the cook/expo line, large knife stored between tables.
Stored food not covered. Observed: in the walk-in cooler, food stored below 41F, not covered.
