Observed dish machine chlorine reading 0ppm. Operator replenished chlorine with 100ppm reading during inspection.
Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed quat reading 0ppm in triple sink. Operator primed with 300ppm quat reading during inspection.

Informed manager to set up 3 compartment sink for washing and sanitizing dishes. Manager set up 3 compartment sink.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced swiss 55F, sliced American cheese 55F, sliced cheddar 56F, cut lettuce 58F, sliced tomatoes 55F, beef patties 55F at 2:35pm. Per Manager, all items placed in cooler drawers at 1:30pm from walk in cooler. Manager voluntarily discarded cut lettuce, removed all other items and placed inside walk in freezer.
Spray bottle containing toxic substance not labeled. Observed 1 unlabeled spray bottle containing a yellow liquid substance. Manager labeled spray bottle as oil.
Employee personal food not properly identified and segregated from food to be served to the public. Observed employee food stored in reach in cooler with food to be served to the public. Manager removed employee food at time of inspection.
Equipment and utensils not properly air-dried - wet nesting. Observed wet cups stored in wet nesting manner at server station.
