Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm). Operator trimmed the chlorine feeder hose, chlorine sanitizer 50ppm.
Spray bottle containing toxic substance not labeled. There is a unlabeled chemical spray bottle in the dish washing area.operator labeled the chemical spray bottle.

Silverware/utensils stored upright with the food-contact surface up. Observed forks and utensils stored on storage rack next to dish machine food contact surface up. Operator moved for cleaning and proper storage.

Observed employee begin touching clean equipment and our gloves on to begin working with food with no hand wash. Educated employee on proper hand washing. Employee washed hands and put gloves to continue working.
Toxic substance/chemical improperly stored. Observed pesticide strip hung on storage rack over bottled food products. Educated operator on proper storage and operator moved for proper storage.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine with a black/green mold-like substance.
