Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed one flying insect in kitchen land on sandwich. Operator immediately threw sandwich away and closed lid on reach in cooler.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the standup reach in cooler;cooked noodles (53F - Cold Holding) red sauce (49F - Cold Holding); cheese (52F - Cold Holding) per operator in and out of cooler more frequently due to additional cooler not in working condition. Ambient air of reach in cooler 39F, discussed storing frequently used product in flip top reach in cooler, operator relocated all items to the reach in freezer, all products under 41F at conclusion of inspection.
Toxic substance/chemical improperly stored. Observed a spray bottle of bleach and one with sanitizer hanging on shelf above container of flour and bag of bread. Operator moved both bottles.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener in kitchen with food debris on prep table by 3 compartment sink. Observed a dirty plastic container with clean utensils stored on table by entrance of kitchen.

Dead roaches on premises. Observed one dead roach on bottom shelf in wait station under pass through window. Employee cleaned up roach and threw away.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In silver reach-in cooler in kitchen, cut tomato (52F - Cold Holding, 2nd temp 40F with 1 hr time lapse); ham (46F - Cold Holding, 2nd temp 38F taken with 1 hr time lapse) Operator placed items in reach-in freezer and brought both temperatures to under 41F.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. On stove top, broccoli cheese soup (93F - Hot Holding) Operator stated soup had been prepared approximately 3 hours earlier and reheated to 176F.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided operator document during inspection.
