Exterior door has a gap at the threshold that opens to the outside. Gaps along threshold of air conditioning units affixed to exterior door to kitchen. Per maintenance manager, door is being replaced in the following week.
Standing water in bottom of reach-in-cooler. Standing water at bottom of both reach-in coolers in kitchen.

Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed two live flying insects in kitchen at pizza make line location.
Operating with an expired Division of Hotels and Restaurants license.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed the following held 3 1/2 hrs- Pepperoni 54F, sausage 54F in pizza make line reach in cooler. Instructed operator to cool down items in walk in cooler or on ice.
A return visit. The problems from the previous inspection had been dealt with.
A return visit while the kitchen was already shut. The problems had not been fixed, so the closure stayed in place. 2 violations recorded, 1 high priority.

Observed approximately twenty-nine rodent droppings in the following locations: approximately ten on floor around drink station in kitchen, approximately five on floor near 3compartment sink in kitchen, approximately three under prep table in kitchen, approximately six on counter near computer in kitchen, one on clean pan on rack in make-line location, one on floor in walk in cooler, and approximately three on floor behind popcorn maker/freezer behind front counter.
Dead roaches on premises. Observed three dead roaches in the following locations: two in tray of hot water heater and one on clean baking pan.
Ordered shut on the spot. 6 violations recorded, 2 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.
