Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where black hose is attached. Operator stated maintenance man will put back flow on today.
Warewashing sanitizing solution exceeding the maximum concentration allowed. In three compartment sink at bar, Triple Sink (Chlorine 200+ppm). Operator filled sink with more water and rechecked at 100ppm.

No paper towels or mechanical hand drying device provided at handwash sink. No hand drying device/paper towels in both men's and women's stalls near gaming area.
