Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : in the walk-in cooler, on site crushed garlic, 55°. This was out earlier today for prep. The food pan was fully wrapped. The garlic was placed in an ice bath for quicker cooling and vented. The walk-in cooler had food in it below 38°.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the soda nozzles on the wait line are soiled. These were cleaned by the staff.
Handwash sink not accessible for employee use at all times. Observed : the hand wash sink on the back prep line was blocked by a rolling cart. The cart was moved.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed : an employee drink stored on the wait line prep table. This was discarded. **Corrected On-Site* ALSO, employee drink rack, with a drink, stored over food service gloves, on the cook line/prep corner.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed : cook line staff zesting limes with a bare hand holding the lime. Gloves were then added for the next process.
Toxic substance/chemical improperly stored. Observed : expo dry erase board cleaner stored next to clean plates. This was moved to cleaning. ALSO, at the bar, a pouch of sani-wipes stored on the counter where garnishes were being prepped. These were moved below.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed : a gallon of milk in the wait station area, no open date. This has been opened since Monday. A date was added.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : 2 tea urns with dried tea residue on the metal part of the spout.
Carbon dioxide/helium tanks not adequately secured. Observed : 3 pressurized tanks behind the bar, not secured.
Employee personal food not properly identified and segregated from food to be served to the public. Observed : a bag with employee drinks stored over guest food in the walk-in cooler. The bag was moved below.
No handwashing sign provided at a hand sink used by food employees. Observed : 2 kitchen hand wash sinks lacking a sign. Signs were made and posted.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink, by the back door and walk-in cooler. This was adjusted/fixed during the inspection.

Dishware/utensils not soaked prior to being placed in three-compartment sink/dishmachine and results in soiled dishware/utensils after washing occurs. Observed: a large cooking pot coming out of the dish wash machine, the bottom was still soiled. Staff moved the pot to be pre-cleaned before going into the dish wash machine again.
Worn, torn and/or soiled floors/carpeting. Observed: the floor behind the door in the nitrous storage room is soiled.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed: the kitchen dish wash machine was at 0 ppm. ALSO, the bar dish wash machine, 0 ppm. Both were primed to 100 ppm.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed: on the cook line, raw beef over dairy and ketchup. The beef was moved below. ALSO, at the bar, raw shell eggs over fringes in the drawer. Eggs were moved below.
Soiled cloths, linen, aprons, coats, or other uniform apparel not kept in a suitable container prior to laundering. Observed: soiled apron on the dry good shelves. This was moved to the laundry basket.
Stored food not covered. Observed croutons in the walk-in cooler, not covered on the cooling rack. Overhead protection was added.
