Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black mold like substance in ice chute at soda dispenser in employee dining room. Operator had employee clean the ice chute.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed handle of scoop in flour in banquet kitchen. Operator moved handle above product.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw smoked salmon in 3 door reach-in cooler the food preparation area over grape tomato's. Operator removed product.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed butter on bottom shelf in food preparation area near mop sink at 70F. Operator stated butter had been there approximately 2 hours. Operator moved butter to walk-in cooler.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phone stored on a preparation table in the bake shop. Employee removed the phone.
Food stored on floor. Several cases of milk stored on the floor in the walk in cooler. Manager placed the items on a shelf.
